Advanced Care Planning: More than a Form
Advance care planning is more than paperwork—it’s a sacred conversation about values, relationships, and what matters most. This presentation describes the importance of advance care planning and documentation and equips the spiritual care provider with tools to assist others in these conversations. Utilizing the Five Wishes framework, this presentation covers: selection and designation of a healthcare agent, or medical power of attorney, discussion about quality of life and life support treatment options and preferences, and end-of-life conversations around comfort and legacy building.
